Part 1 is to be completed by the applicant or project sponsor.Responses become part of the application for approval or funding,
are subject to public review, and may be subject to further verification.
Complete Part 1 based on information currently available. If additional research or investigation would be needed to fully respond to

The Applicant proposes to redevelop the Project Site with a Continuum of Care Retirement Community that
would include 834 units (750 Independent Living and 84 Assisted Living and Memory Care) oriented around
community hubs located within walking distance of the residential units. The Community would include modern
amenity spaces for use by residents, including personal and healthcare services, dining, and recreation. The
Project would include a mix of three-, four-, and five-story buildings, surface and structured parking, and
stormwater management facilities and would be constructed in phases over several years. To facilitate the
Project, the Applicant has petitioned the Village Board for an amendment to the PUD Zoning District and has
requested approval of a PUD Concept Plan.
