On April 1,2022, I witnessed the code required annual no load hydraulic safety test on
one (1) 60001b freight elevator located at the above referenced address. The test was
performed by Otis Elevator.
The elevator was tested to ensure compliance with elevator testing procedures according
to the requirements of ASME A17.1 Safety Code for Escalators and Elevators, as
referenced by the 2020 NYS Building Code, Chapter 30 and Local Codes.
No violations were revealed during the testing of this elevator.
The elevator is deemed safe to use.

The elevator was tested to ensure compliance with elevator testing procedures

However, the following deficiency was revealed that requires correction to comply with
code as listed below:
