NYS QEI Elevator Inspection
Dear Ms. Hakanjin,
This is your letter of compliance certifying that on September 25, 2024, the
elevators located at the referenced address underwent the NYS mandate Category-1
Q E.I. inspection test, in coordination with your elevator maintenance company,
Champion Elevator. The elevators were found to be in good working order and returned
to service. The elevators are in compliance as per NYS IBC 3000.1, PM 606.1, and
ANSUASME A17.1-3 Codes.
This concludes our test/inspection for the year 2024.

NYS QEI Elevator Inspection
Dear Ms. Hakanjin,
This is your letter of compliance certifying that the elevators located at the
referenced address underwent the NYS mandate Category-1 Q.E.I. inspection test, in
coordination with your elevator maintenance company, Champion Elevator. The
violations noted during the initial inspection date of September 28, 2022, have been
remedied. The elevators are now in compliance as of this date.
This inspection test has been conducted according to NYS IBC 3000.1, PM 606.1, and
ANSVASME A17.1-3 Codes. This concludes our testfinspection for the year 2022.

VIOLATION NOTICE

NYS QEI Elevator Inspection
Dear Ms. Hakanjin,
The NYS mandated Q.E.I. Inspection/Test, with a complete test of the smoke
recall system, was performed in coordination with Champion Elevator and the buildings
contracted Alarm Company on September 28, 2022. The elevators located at the
referenced address were found to contain violations and unsatisfactory conditions as
listed below:
Plaza Building 10
1. Render the smoke recall system operational to the NFPA 72 code. The
fire hat in the cab shall be flashing when a smoke condition is detected in
the hoistway and machine room and full on for all other detectors outside
the hoistway and machine room.

2. Render the smoke hatch to open automatically when the hoistway smoke
detector is activated to allow the smoke to be vented out.
Plaza Building 14
1. Render the car door electronic safety edge to be inoperative when in fire
service phase 1 and 2 mode. ( the car door shall be allowed to keep
closing when in fire service)
2. Provide a shunt trip system to be activated by the machine room heat
detector.
3. Render the smoke hatch to open automatically when the hoistway smoke
detector is activated to allow the smoke to be vented out.
4. Render the smoke recall system operational to the NFPA 72 code. The
elevator does not respond to the proper floors and alternate floor. Also the
fire hat bezel does not light up when in fire service.
It shall be necessary to perform a re-inspection when all of the above listed
violations/deficiencies are removed. Have your elevator maintenance company and
alarm company present for the smoke recall test to be performed during this re-
inspection.
As that we are not an enforcement agent, we can only bring to your attention the
deficiencies or violations found during our tesVnspection of your elevator as of
September 28, 2022. it is your responsibility to remedy such deficiencies and or
violations in the set required 30 day allotment. Notify this office when the above has
been taken care of, so we can perform a re-inspection and issue a violation clearance
letter.
This Test/Inspection is in accordance with NYS IBC Section 3000.1 PM 606.1 and the
ANSI/ASME Al7.1- 3 Safety Code for Elevators and Escalators.
