Office Assistant /' v►c JAN ? 2ozo — infinityEnerg0 Mike Izzo, It has been brought to my attention that an employee at Infinity Solar illegally wrote in a false electrical permit number on an inspection request for 3 Fair Lawn Parkway, Rye Brook, NY. Work was also conducted at this house without a permit and also, without my knowledge and consent. I want to be perfectly clear that this was all conducted without informing me of said project and I had no prior to warning or contact from Nick regarding this job. This was done solely without my knowledge. Throughout my years of working as a licensed electrician I have not had any issues nor any complaints. Since my time employed at Infinity Solar as a licensed electrician, we have all strived to ensure that as a company, things are done by the book. I have been licensed in Westchester County for over 20 years and this is the first time something like this has occurred under my watch. I have known Nick for 5+years and throughout our tenure together I have never had any issues in his working under and filing for permits through my license. That being said. this is a massive breach of character and integrity on my part, and for that I express my utmost apologies. This is NOT how I conduct business and will be working closely with Infinity Solar to remedy this situation and set forth measures that will prevent any incident like this from occurring again. I will be working alongside the owner of the company, Murphy Panagiotou, going forward to apply for all electrical permits with him. I have lost my trust in Nick and can assure you that this will not happen again. I sincerely apologize for jeopardizing the safety of the citizens in Rye Brook and give you my word that moving forward all permits and approvals will be obtained prior to the commencement of any work. After this call it was brought to my attention that there was a false building and electrical permit written on an inspection request to State Wide Inspection Services, as well as a failure for us as a company to file and retrieve the electrical permit to install a roof mounted solar panel system. Our Permit Coordinator has briefed me in entirety on the situation at hand and I would first like to apologize on behalf of our company for not following the proper procedure set in forth by the Village of Rye Brook and State of New York. Our Permit Coordinator, Nick, used judgement that is not in conjunction with what we as a company believe in. Nick thought of our customers' satisfaction first and safety and procedures to protect your residents second. Our first priority as a company is to ensure a safe installation for our customers. The misstep in failing to retrieve the approved plans and acquire permission to install the solar panels violates state and local rules, but also violates our company procedures. Nick was thinking only that the customers best interest was expediting their timeline from installation to activation, but he now understands that falsifying the numbers and violating laws that were put in to place to protect your citizens does not serve anyone's best interest. On Wednesday,January 8, Nick drove to the building department to speak with Mike Izzo, building and fire inspector, in an attempt to rectify the situation. In the 6 years of knowing Nick I have never seen him visibly distraught over something that he was in charge of. He knows what he did was wrong, and a violation of the rules set in place. That being said, in an attempt to continue to do business in the Village of Rye Brook, I myself will be applying for all permits and inspections going forward. SWIS has established specific protocols within our organization to ensure that proper documentation is secured prior to performing any electrical inspection. There was a misunderstanding in our office that we received the application for 3 Fairlawn Avenue via the Village of Rye Brook building department,when in fact,the application was submitted to us directly by the electrical contractor who admittedly provided false building and electrical permit numbers. When the electrical contractor called to schedule the inspection,our office pulled the application we had on file and seeing the building and electrical permits numbers, incorrectly assumed the application was processed by the building department and the project was ready for inspection.When in fact,my office should have never accepted the application directly from the electrical contractor in the first place. Since this incident,additional protocols have been established and staff'has been put on notice regarding the severity of this error and the potential impact on their continued employment with the company.Although this was a human error,we pride ourselves with providing service with integrity and are committed to excellence. We recognize the severity of this situation and commit ourselves to the Village of Rye Brook by assigning dedicated staff to all work we perform in the Village of Rye Brook going forward. Our Rye Brook team will report directly to me and inspections for Rye Brook will only be performed by SWIS once my staff has submitted proper documentation to me directly, at which time, I will personally authorize the inspection to be performed.No inspection will be performed by SWIS in the Village of Rye Brook without my direct authorization.
