From: Michael Izzo In our ongoing efforts to promote public safety throughout Rye Brook, should any unsafe or questionable condition be observed during your regular tour of duty,please notify the Building Department using this referral form. Please provide a brief description of the conditions, the location, date&time observed, and forward this form to the Rye Brook Building Department for further investigation and corrective action. Some typical conditions may include: ➢ Unsafe or non-permitted construction activity; unsafe or improper storage/use of construction materials or equipment; unsafe storage of flammable liquids or combustible materials. ➢ Possible residential overcrowding; all rooms converted to bedrooms; questionable basement or attic apartments; multiple kitchens; multiple mailboxes, excessive cars parked on property, ➢ Unsafe or unsanitary conditions; unsafe electrical or mechanical equipment; leaky plumbing; inadequate/inoperable sanitary facilities; excess mold/mildew condition; rodent/insect infestation. ➢ Inadequate fire protection systems; fire alarms &strobes do not sound or flash; missing or damaged smoke detectors; fire sprinkler, standpipe,hose cabinets, ANSI,or other fire protection system damaged or disabled; missing, outdated or discharged portable fire extinguishers. ➢ Exits & exit stairways blocked; exit doors locked; combustible material stored in exit or exit stairway; exit signs not illuminated; inadequate emergency lighting; accumulated snow or ice in exit discharge. -------------------------------------------------------------------------------------------------------------------------------------- Address: 12B RYE RIDGE PLAZA (COCO NAILS) Date:11/5/2022 Time: 20:45 At 20:45 hours we received a commercial fire alarm at the above. Upon arrival we were met with Contractors installing new plumbing and electric for new pedicure stations in the back right area of the occupancy jjftg permitted hours). At some point they pulled or knocked the rear pull station off the wall causing the alarm. We then observed multiple violations inside the occupancy. Taped smoke heads, Blocked means off egress,Exit doors that have improper locking devices and open inwards, Storage blocking the path of egress, extension cords ran thru vents and into the walls in multiple locations, Zip cords used to power appliances, Corrosive and flammable liquids stored in a closet with other combustibles, multiple missing or cracked ceiling tiles, Multiple rooms with combustible storage stacked to the ceiling.
