Z� COUNT ONE: Village Code 6113-6. Electrical Permit Required
TO WIT: In that the above name defendant did allow or cause to allow the electrical work to be performed, at the
above-named premises without the required permits. Said work is including but not limited to demolition of the
existing house electrical wiring, lighting outlets, switches and other components.
COUNT TWO: Village Code 6113-6 Electrical Permit Required.
TO WIT: In that the above-named defendant(s) did allow or cause to allow electrical work to be done at the premises
without permits. Said electrical work is including but not limited to creating an unsafe condition by allowing,
modification to the building main electrical panel and wiring upon a building open and exposed to the public.
OUNT THREE: Village Code 6180-23 Plumbing Permit Required
TO WIT: In that the above-named defendant(s) did allow or cause to allow plumbing work to be done at the subject
,premises prior to the issuance of a required plumbing permit. Said plumbing work is including but not limited to
demolition of the existing building plumbing systems and fixtures.
COUNT FOUR: Village Code 6186-4 Storage of Eauiament.Dumasters and Construction Material.
TO WIT: In that the above-named defendant(s)did allow or cause to allow, a construction dumpster at the premises to
be over filled and flowing over. Said dumpster is causing debris and construction material to be strewn about the site in
violation of code.
All the above is contrary to the provisions of the statutes in such case made and provided. The above allegations of fact are made
by the complainant herein on direct knowledge and upon information and belief provided by Steven E. Fews Code Enforcement
Officer and Building Department Official. Some of which being an inspection(s)of t& subject premises December 1412021
and December 20, 2021 by Code Officer Fews. Any false statements mad a are ble as a s "A" Misdemeanor
pursuant to§2kO.r2a5oofY-P1nalLaw.
